Polymorph

Your form has been changed into the form of another creature. You are subject to the following effects:

• You take on the form of a new creature, replacing all of your statistics (such as attribute scores, attacks, defenses, skills, languages, etc) with the new creature's statistics. Hitpoint damage transfers as a percentage of total Hitpoints of the new form and your Morale does not change.
• You maintain your own memories, personality, and alignment, but they are limited by the mental attributes of the new form.
• Worn and carried equipment falls off or moves to cover the new form if appropriate.
• You retain Status Effects, Injuries and any other effects that would affect this new form (GM discretion may be required.).
• You gain a +20 bonus to Disguise checks to make others believe you are truly the creature you have polymorphed into.
