Polymorph yourself into any
Simple creature of your choice (such as a humanoid race, animal, or beast). The chosen form must have a level less than or equal to your own. When you change into the new form, you retain your own hitpoints and you gain temporary hitpoints equal to the maximum hitpoints of the chosen form. While in the new form you maintain your mental capacity for thought and decision making regardless of the new form's mental attributes. You also retain your languages and skills, and at your discretion alter the form slightly to allow you to speak (though some skills may not be used effectively in the new form). Any gear or equipment (except artifacts and intelligent items) that do not fit on your new body meld into the new form and becomes inert. You do not gain any equipment normally worn by the new form.
Additionally, you may use the following special action to switch between forms:

Effect: You switch between your current form and a new form of your choice. When you do this, this change functions as above and you gain temporary hitpoints equal to the maximum hitpoints of the chosen form.
If your temporary hitpoints are reduced to 0 you are forced back into your normal form. The spell's duration continues and you may use Form Shift to change forms again (gaining more temporary hitpoints).
You may transform into any creature you have seen, including those that are not alive such as a Construct, Undead, or Elemental. The exact appearance of your new form is up to you, as long as it exists within the normal range for the creature's race. Because of this, you may even take the form of specific creature, which grants you a +20 power to Disguise checks made to imitate it.
This spell does not allow you to transform into a Complex or Legendary creature. Additionally, you cannot utilize Shape Change to take the form of an Object (animate or otherwise).
